We recognize that end of life is one of the most difficult events that a person and family must face. We will do everything in our power to earn your trust and we will always strive to be honest and straight-forward in all matters that relate to our patients and family members.
Steward Hospice Care, Inc. is committed to (1) help you to live each day as comfortable as possible and we will make sure to listen to what you want and need, in order to accomplish this goal. (2) ensure that our inter-disciplinary team (IDT) of physicians, nurses, social workers, spiritual counselors, therapists, trained volunteers, and other specialized caregivers will support you every step of the way.
We will visit you regularly to assess your condition, teach you techniques related to personal care, help you address any new symptoms that might arise, and help you plan as your needs change.
We will teach family members care giving and coping skills because we know that the best care you can receive comes from the friends and family who love you.
Our hospice care is constantly available to patients and family – 24 hours a day, 7 days a week. Call us at any time to: make a visit, respond to a sudden change in condition, answer a question, order medication or oxygen or medical equipment, and/or provide emotional or spiritual support.

Steward Hospice provides care wherever people can benefit from hospice care – their home, in a hospital, in an assisted living/ residential care facility, or in a nursing home. Hospice care is covered by Medicare, Medi-Cal and most private insurance; therefore, there are no out-of-pocket expenses for eligible patients or their family.
Our beliefs guide how we conduct our business. We believe:
- Patients have the right of self-determination, autonomy and the right to choose.
- Palliative care is a valid and appropriate choice for persons in any stage of a terminal illness
- Patients and their family determine how the patient will live and die
- We have an obligation to do everything we can, within the boundaries of our ability and resources, to meet the specific needs of our patients and their families.
- We have an obligation to provide the best care and services of which we are capable.
Every 2 weeks the IDT meets to review and update the patient’s Plan of Care. A package of services is available to deliver the Plan of Care, which includes:
- Home Care at the patient’s home, long-term residential care facilities, nursing homes, and assisted-living facilities.
- Continuous/Intensive Care. Steward can provide hospice staff in shifts for a continuous but brief period to help the patient stay at home during a difficult time, rather than transfer to a hospital.
- Inpatient Care. Steward can arrange with local hospitals and other facilities for patients to receive necessary and appropriate inpatient care that addresses needs beyond what can safely be managed at the patient’s home.
- Respite Care. In appropriate cases Steward can arrange for the patient to spend up to 5 consecutive days in an institutional setting, such as a nursing home, in order to provide the primary caregiver(s) a “respite” or “rest”.
In coordination with the patient’s physician, Steward can also offer the following services:
- Drugs, Equipment and Supplies. Steward covers the cost of medications, medical equipment, and supplies required for comfort and safety in treating the patient’s terminal illness.
- 24/7 Care. On-call staff is available to respond to our patients’ needs 24 hours a day, 7 days a week. We’re just a phone call away to attend to questions or respond to a crisis.
- Bereavement. Steward provides trained support for family members whose loved one has died, helping them deal with the experience of loss and grief.
